Ecuador is the entry that tells the whole story. In 2018, its homicide rate was about 5.8 per 100,000 — higher than the United States at the time. By 2023, it reached 45.72. In 2025, InSight Crime's annual homicide round-up recorded a new high of 50.9.
The cause is cocaine. Ecuador sits between the world's two largest coca-producing countries, Colombia and Peru, and its Pacific ports became a transit corridor for shipments to Europe. When trafficking organizations moved in, the violence followed — prison massacres, turf wars between the Choneros and the Lobos, and political assassinations. A country once called an "island of peace" in the Andes became the fourth most homicidal on Earth.
The other surprise is who isn't here. El Salvador had a rate of 103 per 100,000 in 2015 — the highest in the world. Under President Bukele's mass-incarceration crackdown, it fell to 1.3 in 2025. A 98% decline in a decade.

The Western Hemisphere's grip on this list is decades old. Gang activity, drug trafficking corridors, weak judicial institutions, and deep inequality keep Latin America and the Caribbean at a regional homicide average of about 18 per 100,000 — more than three times the global figure. Honduras, once the most violent country on Earth, has improved but sits just below the top 10 at 31.44.
South Africa is the only non-Americas country in the top five. Its 2022 rate of 43.72 reflects unemployment above 30%, one of the world's worst Gini coefficients, and entrenched gang culture in the Western Cape. Lesotho, South Africa's landlocked neighbor at #9, shares many of the same structural pressures on a smaller scale.
The bottom of the global list is predictable. Japan (0.2), Singapore, and Iceland all sit below 0.5 per 100,000. Low inequality, strict gun laws, and effective policing work.
